These programs significantly reduce cardiovascular complications and hospital readmissions. Tailored exercise prescriptions and multidisciplinary care enhance glycemic control and overall outcomes. Patient education and psychological support are key to self-management and adherence. Remote options increase accessibility, while long-term engagement strategies are crucial for sustained health.
