Abstract
Cardiac rehabilitation is a critical, multidisciplinary component of post-heart transplant care. It encompasses exercise training, nutrition, psychosocial support, and education to optimize recovery, functional capacity, and long-term outcomes. Early initiation of supervised exercise, management of psychosocial challenges, personalized nutrition, and patient education on medication adherence and infection prevention are key. These interventions aim to improve graft survival and quality of life, empowering patients for lifelong self-management.
