Abstract
Community-based cardiac rehabilitation (CBCR) programs are critical for improving cardiovascular health and patient selfmanagement. Research indicates CBCR enhances quality of life, reduces hospital readmissions, and offers significant psychological benefits. Success factors include adequate funding, trained personnel, community engagement, and robust referral pathways. Technology integration and the support of community health workers further improve accessibility and adherence. Cost-effectiveness and patient-centered approaches are key advantages, with hybrid models showing promise. Policy and organizational support are vital for widespread implementation and sustainability.
