Abstract
Chronic disease management is enhanced through coordinated, patient-centered integrated health services. Palliative care im-proves life quality for patients and families facing severe illnesses. Telehealth and technology integration support patient adherence and data sharing. Effective management needs patient-centered approaches and addresses social determinants for equitable outcomes.
