Palliative Care: Enhancing Life in Chronic Illness

Abstract
Palliative care improves the quality of life for chronic disease patients through symptom management and holistic support. Early integration in heart failure reduces hospitalizations. Effective communication aligns treatment with patient preferences. Telehealth enhances access and symptom management. Palliative care interventions should be integrated into chronic disease management programs, improving outcomes and reducing healthcare utilization.
